阿片类药物与慢性疼痛:初级保健提供者指南

最后更新:2023年2月

This Guide was created as a resource for primary care providers who may be inheriting patients on opioids or want to provide a higher standard of care for their patients who use drugs.

Providers may also utilize CIAO's Inheriting Patients on Opioids Toolkit here.

The Center for Innovative Academic Detailing on Opioids and Stimulants (CIAO), at the San Francisco Department of Public Health, has prepared online coursework modules with the aim of educating learners on topics related to the management of opioids and chronic pain in the primary care setting as well as how to communicate relevant information to patients and providers.

The knowledge-based curriculum is designed for a wide range of health professionals, including physicians, pharmacists, physician assistants, nurse practitioners, registered nurses, and health educators who care for patients who use opioids, who use illicit substances, or who have chronic pain, in the primary care setting

The curriculum consists of eleven (11) short on-demand courses, which can be completed in any order. A certificate of credit will be issued for each module individually, with an option to print a certificate including all credits earned upon completion of the curriculum. Courses will be released over time, and include:

Currently Available:
1. Naloxone
2. Opioid Use Disorder
3. Buprenorphine
4. Substance Use Disorders
5. Reducing or Stopping Opioids for Chronic Pain
6. Urine Toxicology
7. Opioid Stewardship

8. Starting Opioids for Chronic Pain

Coming Soon:
9. Chronic Pain Management
10. Motivational Interviewing

11. Academic Detailing Basic Skills

This course meets the new Medication Access and Training Expansion (MATE) Act requirements from the Drug Enforcement Administration (DEA) for training on substance use disorders. Please note that not all credit hours are currently available. Providers planning to receive or renew their DEA registration can currently earn 5.75 hours toward their 8-hour MATE requirement. More hours will be added in the near future.

Previous Webinars

The Center for Innovative Academic Detailing on Opioids and Stimulants (CIAO) is continuing its quarterly webinar series on topics related to opioid prescribing, opioid stewardship, managing opioid use disorder, and academic detailing on those topics. The webinar will focus on patients prescribed long-term opioids who are abandoned when a clinic shuts down, or when a clinician relocates or loses their license to practice. The webinar will begin with real case studies about the morbidity, mortality, and challenges faced by patients in this situation, and explore resources available to clinicians who are hesitant to take on either inherited or abandoned patients. A mock detailing session will also illustrate how to talk with clinicians about both the perils of abandoning patients on opioids and some best practices in initiating care for an abandoned patient. The information will be presented using CIAO's Toolkit for Inheriting Patients on Long-term Opioids.

The Center for Innovative Academic Detailing on Opioids and Stimulants (CIAO) is continuing its quarterly webinar series on topics related to opioid prescribing, opioid stewardship, managing opioid use disorder, and academic detailing on those topics. This webinar will briefly review the basics of academic detailing skills and concepts, including performing the needs assessment, preparing for possible barriers, communicating key messages, and supporting clinician behavior change. They will then use reference cases and mock detailing sessions to bridge the gap between theory and application. CIAO’s academic detailing training is modeled after evidence-based practices from the National Resource Center for Academic Detailing.

The Center for Innovative Academic Detailing on Opioids and Stimulants (CIAO) is continuing its quarterly webinar series, CIAO Time, to share evidence-based information related to opioid prescribing, opioid stewardship, and managing opioid use disorder. On February 2, 2022, CIAO staff will discuss the epidemiology of the overdose crisis related to fentanyl and the effect on evidence-based overdose prevention interventions. In particular, CIAO will discuss the impact on naloxone programming and medications for opioid use disorder, including the role of overlap initiation for buprenorphine treatment. This information will be presented using CIAO’s Opioids and Chronic Pain: A Guide for Primary Care Providers guidebook. The presentation will include a live detailing demonstration and an opportunity to ask questions.

Due to recent closures of 29 LAGS pain management clinics in California, up to 20,000 patients receiving pain management will be seeking alternative care. Some of these patients may be on long-term opioid therapy for chronic pain, a serious challenge for nearby providers. If you have received patient transfer requests or are otherwise affected by these closures, please view the following webinar for information and clinical tools about inheriting patients on opioid therapy, ensuring continuity of care, and utilizing buprenorphine. Affected providers can also register for a one-on-one informational session with CIAO’s clinician trainers to discuss these topics at https://www.ciaosf.org/contact-us.
